WELCOME TO H&M CARE

Our primary objective is to give our clients the support they need to remain safely at their home – wherever they call home- for as long as practically possible. We provide long term care for a variety of conditions from Alzheimer’s disease to complex physical disabilities: continuity of care for those who are coming back from Hospital or rehab; supplemental care for those who are residing in Nursing homes and assisted living facilities

APPOINTMENT

EXCEPTIONAL HOME CARE

• Caregiving provided by our own trained, credentialed staff.
• Help with daily living activities, such as eating, hygiene, grooming, and moving around
• Meal preparation and light house keeping
• Dietary monitoring
• Keeping track of and / or taking medication.
• Hourly, overnight, and live in arrangement on a short or long term terms.

Assistance with daily living activities

Help with bathing, dressing, grooming, getting in and out of bed, using the bathroom, eating and other normal everyday functions- at home or as a supplement to a facility care.

Doctor's office visits

We transport and accompany clients to medical and related visits (doctors, rehab, counselling, etc.) ensure flow-through in obtaining prescriptions and adhering to directions, and communicate with family members as appropriate.

Monitoring of Medications and Meals

Help adhering to dietary restrictions and keeping track of and taking medications.

Social companionship

Home care does not have to mean homebound. We provide drivers and companionship to escort clients to dinners, movies, concerts, theater, community events and other social activities that can be so important to an individual’s quality of life and sense of engagement.

Grocery Shopping

We help outline our clients’ nutritional needs and develop shopping lists, then accompany clients to their favorite grocery markets or grocery shop locally for them.

Skilled Nursing Services

Our licensed Registered Nurses perform a variety of highly skilled nursing services as directed by the patient’s physician.They are Caregivers and case managers; they teach,advice& coordinate treatment plans with client, family&team members, and communicate with the client’s physician and family members.
